CHEST SURGERY
It is a surgical method to remove breast tissue from transgender men/non-binary people, and change the size and position of the nipples/areolas to create a more masculine chest contour.
Top surgery is commonly performed for transgender men to create a traditionally masculine chest. However, it is also available to non-binary individuals, lesbian women, and cisgender women who experience chest-related dysphoria.
Top surgery is available not only to transgender men, but also to people of various gender identities and sexual orientations. Therefore, a diagnosis of gender incongruence or prior gender-affirming hormone therapy is not required before top surgery
Top surgery can help reduce breast-related dysphoria and allow patients to express their gender more comfortably. Research has shown that it can significantly improve quality of life, lessen overall gender dysphoria, and positively impact mental health. In addition, it can help prevent issues such as skin irritation, dermatitis, and musculoskeletal problems that may result from long-term binder use.
An incision is made along the border of the areola to remove glandular tissue. This method is typically used when the breasts are relatively smaller and there is no sagging.
For slightly larger breasts with mild sagging, glandular tissue is removed and excess skin around the areola is excised to achieve a flatter contour.
For larger breasts with moderate to significant sagging, an incision is made along the inframammary fold to remove both glandular tissue and excess skin. The nipple and areola are then resized and grafted to a more appropriate position.

While many transgender men seek a traditionally masculine chest shape, non-binary people may have a wider range of goals, making detailed preoperative consultations especially important. During the consultation, you can discuss and decide on factors such as breast size, incision placement, and the size and position of the nipples and areolas. Bringing reference photos of your desired chest shape can also help the medical team better understand your preferences.
